The Medical Report, admitted in evidence as Exhibit P1, states in part: “TO WHOM IT MAY CONCERN MEDICAL REPORT RE: ANIUKWU RICHARD CHINONSO, MALE, AGE:36 YEARS HOSPITAL NO: 444068 The above named Thirty six (36) years old is a patient in our hospital. He is been (sic) managed for pulmonary carcinoma (Lung Cancer) after undergoing chest radiographs computed tomography (CT) scan.
